Digital Navigator - Virtual Care

Digital Navigator - Virtual Care

Reporting to the Manager, Virtual Care, the Digital Navigator, supports and advocates for patients and caregivers providing a robust compliment of services within the virtual care framework. This role acts as both an internal virtual care champion as well as a patient, caregiver and community partner working to achieve high quality standards of practice and workflows to enhance both the patient and the providers virtual care experience. Providing practical training, technical support and troubleshooting for our patients with the focus on inclusivity and accessibility is a critical component to the success of this role. This position will support both LHSC and St. Joseph’s Health Care London virtual care initiatives.

Status: Regular Full-Time

Hours of Work: 37.5 hours per week

Rate of Pay: $27.40 - $34.26 per hour

Qualifications

What you bring:

  • Sucessfull completion of a two (2) Year College Diploma in Information Technology, Administration, Health Administration or as deemed acceptable by the Hospital
  • Minimum two (2) years of relevant, recent experiance
  • Experience working collaboratively with patients/caregivers and community partners in support of virtual care services
  • Recent work experience supporting and implementing initiatives in support of building a more equitable, diverse and inclusive virtual health care environment
  • Demonstrated ability to apply analytical skills for problem identification, solving & solution mapping, building a more effective patient focused virtual care framework
  • Demonstrated experience developing, re-designing and implementing workflows to enhance clinical optimization and patient experience
  • Ability to work with leadership at all levels to implement both strategic and operational change
  • Excellent change management, customer service, and organizational skills
  • Ability to support digital care technology within the virtual care framework
  • Excellent written, oral, presentation, and listening communication skills
  • Proven ability to work independently and collaborate with members of a team
  • Demonstrated ability to prioritize multiple competing priorities and changing demands that require attention to detail
  • Excellent interpersonal skills to establish and maintain effective working relationships
  • Proficiency in French or Arabic would be an asset
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC’s Mission, Vision and Values
  • Demonstrated ability to attend work on a regular basis

Immunization Requirements:

  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis. 
  • Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
